Red light therapy — also called photobiomodulation or low-level light therapy — has moved from clinical settings into the consumer market fast. Panels, masks, wands and handheld units are now widely sold, with claims attached to them ranging from the plausible to the frankly unsupportable.

This article covers what the underlying science actually says, where the evidence is genuinely early, and — importantly for anyone considering a purchase — how to evaluate a device and the claims made about it.

The Mechanism, Which Is Real and Rather Elegant

The proposed mechanism is specific and well characterised, which is unusual in wellness.

Red light, roughly 620 to 700 nanometres, and near-infrared light, roughly 700 to 1000 nanometres, penetrate tissue to varying depths. At those wavelengths, light is absorbed by cytochrome c oxidase, an enzyme in the mitochondrial electron transport chain — the final step in how your cells produce energy.

The proposal is that photon absorption at this enzyme influences its activity, affecting the rate of ATP production. Downstream, changes in nitric oxide signalling and reactive oxygen species have been described.

This is a genuine, physically plausible mechanism rather than hand-waving. Absorption at these wavelengths is measurable in the laboratory. That is the strongest part of the case.

What is far less settled is the step from “this enzyme absorbs this light” to “therefore this device produces this outcome in your body.” That leap is where most consumer marketing lives, and it is a bigger leap than it is usually made to sound.

Where the Evidence Actually Stands

Photobiomodulation has been studied for decades, and the literature is large. It is also uneven, and honest summaries of it tend to say the same things.

Skin is the most-studied application. Research on skin appearance and collagen has generally been more encouraging than research in other areas, and this is where clinical dermatology has taken the most interest.

Study quality varies widely. Many trials are small. Wavelengths, power output, treatment duration and total dose differ so much between studies that comparing them is genuinely difficult. A result obtained at one dose says little about a device delivering a different one.

Dose matters and is poorly standardised. There is reason to think the relationship is biphasic — that more is not simply better, and that beyond a certain point additional exposure adds nothing or may work against you. Consumer devices rarely give you the information needed to know where you sit on that curve.

Effects, where found, tend to be modest. This is a supplementary practice, not a transformative one.

I have deliberately not reproduced specific percentage figures from individual studies here. Those numbers circulate widely in online coverage, frequently detached from the population, dose and endpoint that produced them, and often altered in transit. A number without its study attached is not evidence.

What This Article Will Not Tell You

Red light therapy is marketed for a long list of medical conditions — joint pain, skin conditions, hair loss, wound healing, cognitive decline. You will find confident percentages attached to each of them online.

This article does not make those claims, for two reasons.

The first is that the evidence for most of them is early, mixed, or drawn from small trials that have not been reliably replicated. Presenting early findings as settled does readers a disservice.

The second is legal, and it is worth understanding as a consumer. In Australia, a device marketed for a therapeutic purpose is a therapeutic good, regulated by the Therapeutic Goods Administration. Devices making therapeutic claims are generally required to be included in the Australian Register of Therapeutic Goods, and there are rules about what may be advertised to the public — with particular restrictions around serious conditions.

This is directly useful to you as a buyer: a device sold with bold claims about treating a medical condition, from a seller who cannot point to appropriate regulatory status, is telling you something about that seller.

If you have a medical condition, the question of whether light therapy has a role in managing it is one for your doctor. It is not a question a product page can answer, and it is not one this article can answer either.

If You’re Considering a Device

Whatever you make of the evidence, some purchasing considerations are straightforward.

Wavelength should be specified. A device that does not tell you what wavelengths it emits, in nanometres, is not giving you the most basic specification.

Irradiance should be specified, with a distance. Power density — usually milliwatts per square centimetre — is what determines dose, and it falls off sharply with distance. “High power” as a marketing phrase is meaningless. A figure without a stated distance is close to meaningless too.

Treatment area matters. A small handheld unit and a full panel are different products for different purposes, whatever their wavelengths.

Be sceptical of condition claims. See above. The boldness of the claim is often inversely related to the quality of the evidence behind it.

Eye protection. Bright sources should not be stared into. Many devices ship with goggles; use them.

Safety

The safety profile of red and near-infrared light at consumer exposure levels is generally favourable. There is no ultraviolet component, so the DNA-damage pathway associated with UV exposure does not apply. Reported adverse effects tend to be minor — occasional headache, eye discomfort, mild skin warmth.

The main cautions worth knowing:

  • Photosensitising medications. A number of common drugs increase light sensitivity. If you take regular medication, check with your pharmacist or doctor.
  • Eye exposure. Do not look directly into the source.
  • Existing skin conditions. Speak to your doctor before treating affected areas.
  • Pregnancy. Evidence is limited; discuss it with your healthcare provider.

The Honest Summary

Red light therapy has a real, specific, physically plausible mechanism, a large but uneven literature, and a consumer market that has raced ahead of the evidence.

The most reasonable reading is this: skin appearance is where the research is strongest; effects where they exist are modest rather than dramatic; dose matters and is poorly standardised across consumer devices; and the confident medical claims attached to many products are not supported by the state of the evidence.

If you want to try it for skin, buy a device that publishes its wavelengths and irradiance, use it consistently, and hold modest expectations. If you are looking at it for a medical condition, that conversation belongs with your doctor.
